Abstract
Acute basophilic leukemia (WHO-Classification 2008-Acute myeloblastic leukemia-NOS) is a rare disease that represents less than 1% of all the cases of acute myeloblastic leukemia. We report the case of a 70 year old man, who presented weight loss, massive splenomegaly and 90.000 leukocytes/ mm3, with 80% of blasts of medium size and low number of granules. Cytochemistry reactions were negative for peroxidase and Naphtol AS D Esterase, and weakly positive for periodic acid–Schiff. Bone marrow’s immuno - phenotype didn’t allow lineage assignment: lymphoid markers were negative, HLADR++; CD13-/+; CD33-/+; CD34++; CD123+/++. Translocation t(9;22) was negative. A tolouidine blue staining showed a positive metachromatic reaction, allowing thus a diagnosis of acute leukemia with basophilic differentiation.
